About Female To Male

Gender dysphoria, the experience of incongruence between biological sex and gender identity, is not a mental illness. The most effective treatment combines counselling, hormone therapy, and, for those who choose it, surgery. Some individuals are well served by hormones and counselling alone; others choose to proceed through surgical stages. We do not push patients towards surgery, and irreversible procedures require appropriate referrals.

Female to male gender reassignment at our centre is a staged pathway. Most patients begin with chest reconstruction, also called top surgery, where breast tissue is removed and a male chest contour is created. Hysterectomy and oophorectomy follow with gynaecology colleagues. Phalloplasty or metoidioplasty and scrotal reconstruction are the most complex stages and are planned later in the pathway.

Hormone therapy plays a central role in transgender FTM surgery. Testosterone, taken orally or by injection, drives many of the visible male changes including voice deepening, body hair, and muscle distribution. Some individuals find hormone therapy sufficient. Others choose to proceed to surgical masculinization surgery, including chest, genital, and facial procedures, in stages over 12 to 24 months.

During consultation Dr. Agarwal reviews medical history, hormone status, mental health and endocrinology referrals, and personal goals. Each stage of gender transition surgery FTM is explained in detail with realistic outcomes, recovery times, and risks. Photographs are taken with consent and stored on encrypted systems.
