About Oral Submucous Fibrosis (OSMF)

Oral Submucous Fibrosis, or OSMF, is a chronic condition in which the lining of the mouth becomes progressively stiff and fibrosed. As scar tissue builds up, mouth opening narrows and patients struggle to chew, brush their teeth or even speak comfortably. It is widely recognised as a precancerous disorder, so timely diagnosis and treatment are extremely important.

OSMF usually begins at the back of the mouth and slowly extends forward, eventually involving the cheeks, palate, tongue and lips. Three overlapping phases are recognised: an early inflammatory stage with burning and ulceration, a fibrotic stage with pale, blanched mucosa and palpable bands, and a late trismus stage in which fibrous bands severely restrict jaw opening.

The dominant cause is repeated tobacco exposure, whether as gutka, pan masala, betel quid or smoking. Areca nut and slaked lime are particularly damaging. Prevention starts with stopping all forms of tobacco and areca, improving oral hygiene and ensuring a balanced diet with adequate iron, vitamins and antioxidants. Untreated OSMF can lead to anaemia, dental disease, voice change and oral cancer.

Severity is graded clinically. Grade I shows a burning sensation without fibrous bands. Grade II adds visible bands and a mouth opening of 26 to 35 mm. In Grade III, blanched mucosa and tongue involvement appear with mouth opening between 6 and 25 mm. In Grade IV, fibrosis extends to the lips and mouth opening falls below 5 mm.
