About Tissue Expansion

Tissue expansion surgery in Lucknow is used to address wide or unsightly scars, post-burn alopecia, soft tissue defects and certain reconstructive needs of the breast and trunk. The principle is simple: a silicone balloon-like expander is placed beneath healthy skin next to the area of concern, then gradually inflated over several weeks. The skin overlying the expander stretches and grows, giving the surgeon a generous flap of natural, hair-bearing or colour-matched tissue to use later for reconstruction.

Plastic surgery skin expansion is delivered in two surgical stages. In the first stage, the expander is placed through a small incision under general or regional anaesthesia. After healing, the patient returns weekly for a few minutes to have a small volume of saline injected through a buried port, gradually inflating the device. The total duration of inflation depends on the size of skin needed, typically six to twelve weeks. The patient learns to manage the visible swelling discreetly under loose clothing.

In the second surgical stage, performed once enough new skin has been generated, the expander is removed and the scarred or affected area is excised. The freshly grown skin is then advanced or rotated into the defect and stitched into place. Because the new skin matches the colour, texture and hair pattern of the surrounding area, results blend more naturally than skin grafts. Scalp tissue expansion is especially valuable for restoring hair-bearing skin after burns or trauma.

Soft tissue reconstruction with expanders also has a role in breast reconstruction after mastectomy, where an expander is placed beneath the chest skin and gradually inflated to prepare a pocket for an implant or autologous flap. Dr. Agarwal, FRCS Edinburgh, evaluates each case carefully, since tissue expansion has clear advantages but also requires patience, regular hospital visits and acceptance of a temporarily visible bulge during the inflation phase. The trade-off is well worth it for many reconstructive scenarios.

Common questions

Frequently asked questions

Is tissue expansion painful?
The initial implant placement is performed under anaesthesia and is well tolerated. Weekly inflations cause a tight sensation for a few hours, but most patients return to daily life immediately afterwards. Mild discomfort is managed with simple painkillers. Detailed pain control plans are shared after each stage. Severe pain is uncommon and should be reported to the team.
How long does the whole process take?
From first surgery to final reconstruction, tissue expansion typically takes three to six months. The expander is inflated weekly over six to twelve weeks until enough new skin is generated. After a brief consolidation period, the second-stage surgery removes the expander and replaces the scarred area. Dr. Agarwal, FRCS Edinburgh, shares a written timeline at the start.
What does tissue expansion treat?
Tissue expansion treats wide scars on the face, neck and trunk, post-burn alopecia of the scalp, soft tissue defects from trauma or surgery, certain congenital deformities and is part of breast reconstruction after mastectomy. It is also used to grow donor skin for microvascular transfer. Suitability depends on local tissue quality, size of the area and overall health.
Is tissue expansion safe?
Tissue expansion has a long safety record when performed in experienced hands. Common issues are minor and include mild discomfort during inflation, temporary visible bulging and rare expander leakage or infection. We screen each patient carefully, plan expander size and position to fit anatomy and provide weekly review during inflation. Severe complications are uncommon and managed promptly.
Are there any drawbacks I should know about?
The main drawbacks are time and visibility during inflation. Patients need weekly clinic visits for several weeks, and the expanded area appears progressively larger until the second-stage surgery. Loose clothing and a bit of patience help most patients through this phase. The trade-off is access to natural, well-matched skin that grafts cannot match for reconstructing the area.
