About Sciatic Nerve Injuries

The sciatic nerve branches from the lower back, runs through the hip and thigh, and provides sensory and motor function to the leg and foot. The common peroneal nerve, originating from the thigh, controls leg muscles and supplies sensation to parts of the leg and foot. Injuries here cause weakness, sensory loss, foot drop and chronic pain, demanding focused lower limb nerve injury care.

If the sciatic nerve is damaged or cut, ideally, surgical excision of the damaged segment with nerve repair or grafting should be done as early as possible, generally within three months of injury for the best outcome. When no clear injury exists but symptoms appear gradually, other neurological causes or compression by tumour must be investigated. In such cases, nerve release with biopsy of fascicle and muscle may confirm a neurological diagnosis.

The common peroneal nerve can be injured by direct accidents, compression or stretching, particularly during fractures or dislocations of the knee and leg. Injury causes inability to lift the foot, leading to foot drop, sensory loss over the front and outer leg and altered walking pattern. Sciatica treatment in Lucknow at Kayakriti includes both surgical and rehabilitation strategies tailored to nerve and patient.

If the peroneal nerve is cut, surgical repair or grafting is best done within six months of the injury for optimal recovery. When patients present late, beyond six to twelve months, tendon transfers become the mainstay. A functioning tendon from the inner side of the leg is rerouted and sutured to tendons on the front of the foot and toes, restoring extension and a near-normal walking style.

What to expect

  • Detailed history of injury, weakness pattern and pain distribution.
  • Examination of foot drop, sensation, reflexes and gait.
  • Nerve conduction studies, EMG and MRI as appropriate.
  • Microsurgical nerve repair or grafting in early presentations.
  • Tendon transfers planned for late or non-reconstructable nerve injuries.

Recovery

  • Hospital stay is usually two to four days for major nerve surgery.
  • Walking with support resumes within two to three weeks of surgery.
  • Desk work returns in four to six weeks for most patients.
  • Final foot strength and gait improvement evolve over six to twelve months.
