Temporomandibular joint ankylosis occurs when the condyle of the mandible fuses with the glenoid fossa of the temporal bone. This obliterates the normal articulation, leaving the lower jaw immobile and the patient unable to open the mouth properly. Eating, dental hygiene and speech all suffer. Normal mouth opening, measured between the upper and lower front teeth, is around 4 to 5 centimetres. In TMJ ankylosis, this can drop to a few millimetres or even zero. The condition may affect one or both joints, leading to facial asymmetry on the affected side. Children are particularly vulnerable because ongoing jaw growth is arrested, causing progressive deformity if jaw joint stiffness treatment is delayed.

TMJ ankylosis usually follows trauma to the joint, inflammatory arthritis, ear infections or post-operative complications of earlier TMJ surgery. Many families consult several physicians before a correct diagnosis is reached. Diagnosis is largely clinical, supported by 3D CT imaging of the joint. True intra-articular ankylosis must be carefully distinguished from extra-articular causes of restricted opening, since management strategies differ significantly.
